Terms and Conditions

It is legal requirement following the Veterinary Surgeons Act 1966, for any animal receiving treatment for lameness, rehabilitation, injury or dysfunction to have a signed and dated veterinary consent referral form provided to Charlotte Goodall Veterinary Physiotherapy prior to the initial assessment. (This can be obtained for the referral form on the website or over email).  

Obligations to you 

  •  I will observe and abide by the codes of conduct supplied by the NAVP and RAMP professional registers. 

  • I ensure professional insurance and membership is updated regularly 

  • I employ the required health and safety measure when treating the animal and whilst on your property 

  • I endeavour to get a report delivered to you and your vet within 5 days following treatment. 

Your Obligations to Charlotte Goodall Veterinary physiotherapy. 

  • When signing the referral form you are confirming you are the legal owner or loner of the animal referred for treatment. You are ensuring information on the consent form is correct and you are agreeing for your animal to undergo treatment. 

  • When signing you are allowing CG vet physio onto your premises during booked appointments. 

  • You are ensuring the animal is clean and fully prepared for treatment. (We ask that the animal has be caught and cleaned if dirty prior to arrival of the physio.)

  • You take responsibility in declaring any health and safety issues about your animal to the therapist prior to treatment. 

  • You understand that Charlotte Goodall Veterinary Physiotherapy reserves the rights to refuse treatment of your animal and will refer the animal back to the veterinarian if the condition deteriorates or a new lameness arises. 

  • You must inform CG vet physio if your animals condition deteriorates during the course of treatment between sessions.

Insurance

NAVP recognised by insurance companies however, I ask that you complete your own forms directly with your insurance company. (I am happy sign any relevant documentation).

Cancellation Policy

All cancellations of appointments more than 24 hours in advance will not incur a charge.

Cancellations within 24 hours of the appointment will incur and 25% charge of the appointment fee. 

In the case of emergency if a cancellation is requested within 24 hours of the appointment the charge may be dismissed dependent on the severity of the event.

YOU WILL NOT BE CHARGED AND MUST CANCEL YOUR APPOINTMENT IF YOU OR YOUR ANIMAL IS SUFFERING FROM A CONTAGIOUS DISEASE OR SICKNESS PRIOR TO THE APPOINTMENT. 

If you are experiencing any symptoms of COVID-19 and are required to self-isolate you must inform CG vet physio and cancel your appointment. No charge will be incurred.

Payment policy

  • Charges for the services are noted as standard on the website. Changes in charges may be made dependent on type of treatment and distance travelled, however this will be made aware to the owner prior to the appointment. 

  • Please enquire for group discounts 

  • Services must be paid in full on the day of or prior to the appointment unless agreed otherwise. 

  • Payment is accepted in the form of cash or bank transfer.
